How the Inpatient Wound Care Program Works
The WoundCentrics’ Inpatient Program provides a comprehensive
evidence-based solution to hospitals for all wound care needs.
PROVIDERS
WoundCentrics will manage and credential a wound care specialist that will collaborate with facility medical staff and the wound care nursing team to optimize the plan of care for each patient’s individual needs.
WoundCentrics’ providers will round in the hospital, provide stewardship, oversight, and perform advanced modalities including bedside debridements.
WoundCentrics’ provider will eliminate the need for wound care nursing staff to obtain telephone or verbal wound care orders from non-wound care trained physicians.
WoundCentrics’ providers manage high acuity wound care patients and facilitate coordination care and promote the goals of case management for discharge planning.
WoundCentrics’ providers participate in the case management process by attending or providing written updates to the IDT team and can facilitate written orders needed for discharge and help coordinate the patient for follow-up in the hospital outpatient wound center.
NURSING PROGRAM
WoundCentrics’ wound care nurses will assess every new patient, and identify wounds that are present prior to admission, adding to the level of care patients receive. Patients will receive a head-to-toe skin and wound assessment from your WoundCentrics’ wound care team with recommendations for high-risk patients.
WoundCentrics’ wound care nurses are trained and specialize in the staging of pressure injuries. An accurate staging at initial assessment helps to identify necessary diagnostic tests to establish a baseline and monitor for underlying chronic conditions and nutritional status that impact and impede wound healing.
WoundCentrics’ wound care nurses collaborate with wound care providers and other specialists to tailor a plan of care specific to the individual patient’s needs. Pressure injury prevention including interventions related to pressure redistribution, nutrition, mobility, repositioning, moisture and shear management, and patient and caregiver education.
WoundCentrics’ wound care nursing program integrates with hospital policies, protocols, and case management to ensure quality and compliance including documentation and billing requirements.
WoundCentrics Providers are the Specialist in Wound Care
From an administrative perspective, the corporate team from WoundCentrics will provide oversight, quality measures, and evidence-based wound care best practices.
WoundCentrics’ providers and wound nurses have access to our clinical support staff to review photos and discuss problematic wounds.
WoundCentrics’ clinical support team also reviews documentation to ensure that appropriate debridement(s) are completed in a manner (timely and sufficiently aggressive) to promote optimal outcomes.
WoundCentrics’ corporate team will also review provider documentation and facility coding to ensure appropriate diagnosis, level of care, and procedure codes for accurate reimbursement.
WoundCentrics’ providers can follow patients post-discharge to post-acute facilities to ensure continued wound healing and prevent possible readmissions.
