We offer a wound care service program designed to provide advanced wound care in the Skilled Nursing Facility.

Providing wound care to patients in the Skilled Nursing Facility requires expertise in wound care and an understanding of the rules and requirements of the SNF as well as the unique challenges this setting provides.

Private Post Acute WoundCare

We train our medical professionals on how to provide post-acute wound care that specifically addresses the challenges that face the administrative, clinical and financial needs of the facility.

Our wound services program offers you both clinical advantages and risk management strategies that will produce outcomes superior to those of your competition.

• WoundCentrics’ wound care provider(s) (*a physician and/or mid-level practitioner) will be credentialed to provide wound care services at your facility. An Agreement is optional at your discretion.

• Our provider will round on-site weekly or more often as needed and coordinate wound care plans of care and treatment with your team.

• Our providers are skilled to perform nearly all procedures at the bedside with your wound care team including sharp debridement; Our providers are trained to work within your formulary of wound care dressings and products to provide cost effective care;

• Our providers will coordinate treatment with your nursing staff and administrative oversight

• Our providers will follow all wound consults requested, regardless of payor status;

• There is no fee to the facility for our service, we bill the patient’s insurance directly;

• Our providers manage all types of wounds including; Pressure, Diabetic, Vascular, Traumatic, Post-operative;

• We offer an optional, secure cloud-based, HIPPA compliant wound imaging software for wound measurements and photo-documentation which allows for remote support for your staff when the provider is not present in the building;

• Our clinical support team can provide educational presentations and training to your clinical staff on a variety of wound care topics.
